Group Enrollment Form

Description

This form is to record group totals for youths involved in special 4-H programs. These include special interest sessions such as school enrichment and instructional TV.

All participants in these programs need not be members of organized 4-H clubs. All participants are, however, counted for ES-237 statistics. Those participants who are 4-H members are considered duplicates in this file.
